Genital fat grafting, also known as autologous fat transfer, is a technique that uses the patient’s own tissue to restore youthful volume to the labia majora and mons pubis — naturally, and with long-lasting results.

Ageing, weight loss and hormonal changes gradually deplete the subcutaneous fat that gives the external genitalia their youthful contour. The result can be deflation, wrinkling and reduced cushioning of the labia majora. Because the transferred tissue is autologous, there is no risk of allergy or rejection — and the graft brings stem-cell-rich regenerative potential with it.

The Three Stages of the Procedure

First, fat is harvested atraumatically — typically from the abdomen or inner thighs — using fine cannulas under local anaesthesia. Second, the graft is processed by decantation, filtration or low-speed centrifugation to concentrate viable adipocytes. Third, the purified fat is injected in fine layers, building smooth, symmetrical volume.

“Fat grafting rewards patience: place small parcels in the right plane, and the tissue does the rest.”

Approximately 60 to 80 percent of the transferred volume survives long term, which is why slight overcorrection is standard practice. Combining the procedure with PRP may further support graft survival and skin quality — a protocol explored in detail in our regenerative masterclasses.

Recovery and Expectations

Swelling and bruising settle over one to two weeks; final results are assessed at three months. Patients appreciate that the outcome feels entirely natural — because it is. For clinicians, the learning curve centres on gentle harvest, meticulous processing and conservative, layered injection: exactly the workflow taught step by step in ITCGA’s dedicated masterclass.
